Tips for Completing the Parent Packet
When prompted to change your password, please write down this new password. If you try to log in using the old password, your log in will fail. If you do not change the password when prompted to do so, the old password will NOT be saved. Without a password, you will not be able to log in to complete the parent packet.
When reading the parent information, please print out any and all information that you feel is important. One recommended page to definitely print out is the Student Packing List.
Before starting to complete the Health History Form, please ensure that you have the following information:
Emergency Contact Information
Health Insurance Information
Immunization Records
Medication Information including dosage and frequency
When completing the Health History Form, be aware that all the fields noted with a red asterisk are mandatory fields. These fields must be completed to submit your child’s Health History Form.
If you cannot complete the health history form for any reason, please click the “Save & Next” button at the bottom of the page. Due to the sensitive medical information on the health forms, there is an automatic time out feature for discontinued use. If you are timed out and did not save the information, you will lose that information.
Prior to submitting your child’s health form, please print out a copy for your records. If for any reason your child’s form does not submit electronically, you will have this paper copy to turn in.
